Facts

The claimant had long-standing degenerative disc disease. She presented to the Medical Clinic with classic lower back strain symptoms. She was instructed to follow up with her primary physician. She did not. A year later, she went to urgent care with a request for weight loss pills. She also complained of lower back pain, and was again advised to see her primary physician. She did not and instead changed her health plan membership to another medical plan. Several months later, she underwent surgery for thoracic spinal stenosis at several levels. Three surgeries were required.
